Full Forum registration fees include admission to all Forum sessions and workshops, session materials, group meals (approximately two meals per day), receptions, and special events.

Payment and Cancellation Policies

Registration cannot be processed without a check, purchase order or credit card payment. All registration changes, cancellations, substitutions or requests for refunds must be submitted in writing. Please fax registration changes to (212) 460-5460.

There is a $75 nonrefundable cancellation fee through September 30, 2011. No refunds will be granted thereafter. If you are unable to attend, substitutes are accepted.

Registrants receive a written registration confirmation. Please call (800) 787-7477, ext 1 if you do not receive a confirmation within three weeks of your registration. Confirmation is important for the reservation of space and materials.
